You must submit your cases on OAA approved report forms. You may download and print these pdf forms and use them for your presentation but it is highly recommended that you download and use the Excel forms for your case reports. This spreadsheet contains both the initial and follow-up forms.

Before attending a fellowship meeting each candidate must :

• Fill out the application form and answer all questions that apply
• Submit these forms to the OAA main headquarters at the address listed at the end of these forms.
• A fee of $450 US must accompany the application.
• Receive a fellowship Identification number which is used during testing in place of candidates name.
• Successfully complete a ten minute phone interview.
